The Visa Application Form (Formulario de Solicitud de Visa or FSV, in spanish) is an essential requirement to begin the visa application process. 

Kindly note that the FSV is a SWORN DECLARATION.

Applicants are requested to:

- Complete the FSV, preferably by a computer (PC);
- Fill in ALL the fields of the FSV. In those fields where there is no information to provide, you may write "NO". 
- FSV's that are crossed out or corrected with liquid corrector will not be accepted;
- the FSV must be signed, with the name clarified in pinyin.
